In-person consent form required.

Due to Florida law, all trans adults must sign a state-mandated consent form in person with a licensed MD or DO before starting gender-affirming hormone therapy (GAHT).

This applies to all Florida patients, whether you’re new to gender-affirming care or a returning patient. The clinician who signs your consent form is the one who will prescribe your GAHT. Once this step is complete, you can begin receiving GAHT from that clinician.

Plume Clinic hosts monthly in-person consent signing events across Florida to help you meet this requirement.

What This Means for You

After completing your Get Started form:

  • Setup your Plume login and follow the instructions on your Plume Member Dashboard to complete your intake.
  • Schedule your initial consultation (this is not the in-person visit).
  • After your consultation, you’ll schedule your required in-person visit.
  • Once your in-person consent is completed, you can begin GAHT care.
  • Log in to your Spruce account and message us, or email hello@getplume.co.
  • We’ll help reactivate your membership and share details about our next in-person consent signing events.

Once your in-person consent is completed, your care can resume virtually.
