How do you start HRT in Alabama?

In Alabama, you generally need to be 19 or older to access gender-affirming hormone therapy independently. That’s because Alabama’s 2022 law, SB 184, applies to people under 19. Adults can still access HRT through a licensed provider.

Not entirely. Alabama banned certain forms of gender-affirming medical care for people under 19 under SB 184 / Act 2022-289, first passed in April 2022. That law remains in effect as of March 2026. It does not ban gender-affirming hormone therapy for adults 19 and older.

Alabama requires at least one in-person visit per year to start and maintain a testosterone prescription, meaning fully remote telehealth clinicians usually cannot offer this care in the state. While Alabama law does not require therapy before starting testosterone, clinicians may have their own clinical processes to ensure treatment is appropriate for you. Many follow an informed consent model, where care is based on your goals, medical history, and understanding of treatment.
If you’re 19 or older, testosterone can still be prescribed in Alabama by a licensed clinician. The first step is typically an appointment to review your goals, medical history, and eligibility for treatment. Alabama’s legal restriction applies to people under 19, not adults.
Alabama’s SB 184 restricts certain gender-affirming medical and surgical care for people under 19. It is not an adult ban. For adults, access to surgery depends more on provider availability, referral requirements, and where the procedure is performed.
For an Alabama driver’s license, current guidance says you usually need either an updated birth certificate or a surgeon’s letter stating you have had “complete and irreversible” surgery. That requirement remained in place after a September 2024 Eleventh Circuit ruling. A separate 2025 law, SB 79, also tightened Alabama’s broader legal framework around sex being recorded as male or female as observed at birth.
