Trans History Spotlight: Bottom Surgery & Dora Rudolfine Richter

We must remember and share our history. It’s important to tell the stories of our diverse, rich community. That’s why we’re excited to launch the Trans History Spotlight! It’s a new monthly series that will explore the key moments, figures, and milestones of transgender history. Each month, we’ll explore some of the medical breakthroughs that have helped our community. We’ll also look at key cultural moments that left a lasting impact. This series is all about celebrating where we’ve been, recognizing how far we’ve come, and honoring the stories that have helped us form who we are today.

This month’s spotlight: Dora Rudolfine Richter and the history of trans bottom surgery.

The First Documented Gender-Affirming Surgery: Dora Rudolfine Richter

Dora Rudolfine Richter was a trailblazer in transgender history. Born in Germany, Dora’s story is one of incredible bravery and resilience, supported by pioneering doctors who helped her through her transition.

Dora underwent the first documented gender-affirming surgery under the care of Dr. Magnus Hirschfeld, a major advocate for LGBTQ+ rights and the founder of the Institut für Sexualwissenschaft (Institute for Sexual Research) in Berlin. First, Richter underwent an orchiectomy in 1922. An orchiectomy is a procedure where the testicles are surgically removed. Then a penectomy – which involves the removal of the penis – and vaginoplasty in 1931.

The vaginoplasty is a reconstructive surgery that creates a new vagina, using tissue from other parts of the body. These procedures were not only groundbreaking medical achievements but also pivotal moments in the advancement of transgender healthcare. Her relationship with Dr. Hirschfeld is an early example of how medical professionals can collaborate with transgender individuals to provide respectful and effective care. This partnership, rooted in understanding and empathy, is a cornerstone of today’s approach to gender-affirming healthcare.

Dora Richter’s story, despite being over a century old, symbolizes how far we’ve come in the realm of transgender healthcare and surgery. Dora’s surgeries marked significant medical progress and provided much-needed visibility for the transgender community. At a time when transgender issues were poorly understood, her journey offered a hopeful narrative and demonstrated that medical science could affirm gender identity. By reflecting on her legacy, we not only honor her contributions but also reaffirm our commitment to transgender healthcare and helping trans people thrive in their authenticity.
